Edit: The backstory is as follows.
John is hugging Karkat from behind, because he surprised him (there’s no way he can manage to hug him otherwise…). Karkat faught, but John was resilient, so he eventually gave up. With John nuzzling him, he got all flustered by his feelings, but he doesn’t want to awknowledge red feelings towards a human, so that why is face is all slightly deranged. He doesn’t want to admit or understand his own feelings at the moment, but he’s still happy nonetheless.
